Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Northwell Health. As a Physical Therapist, you will be responsible for evaluating and treating patients with disabilities, injuries, and/or diseases using physical, chemical, and mechanical means.
Key Responsibilities- Evaluate patients to determine the extent of their disabilities or impairments and develop treatment plans to achieve established goals.
- Administer direct patient care in accordance with written prescriptions and/or referrals from licensed physicians or authorized practitioners.
- Maintain complete medical records for each patient, including documentation of treatment plans, progress, and outcomes.
- Participate in quality improvement chart reviews and activities to ensure high-quality patient care.
- Collaborate with interdisciplinary staff to develop and implement comprehensive treatment plans.
- Master's Degree in Physical Therapy or equivalent combination of education and experience.
- Current license to practice as a Physical Therapist in New York State, plus specialized certifications as needed.
- 1-3 years of relevant experience in physical therapy.
