Hospice care is characterized by a collaborative approach that encompasses expert medical treatment, effective symptom and pain management, as well as emotional and spiritual support tailored specifically to the needs of the patient. Our primary objective is to honor our patients' wishes while providing support to their families and caregivers. Hospice services aim to help patients maintain the highest possible quality of life. We are committed to caring for the entire individual, rather than solely focusing on the illness.
Additionally, Dignity Health Hospice offers bereavement services for a full year following a loss. Our trained bereavement team is available to assist families and caregivers in coping with the grief of losing a loved one.
Responsibilities
Position Summary
In alignment with the Marian Medical philosophy, the Clinical Pharmacist is tasked with the procurement, manufacturing, compounding, dispensing, distribution, storage, and administration of medications, biologicals, and chemicals in accordance with established professional standards, state and federal regulations, as well as departmental policies and procedures.
This role includes providing drug information to other healthcare professionals, patients, their families, and the public. The ability to exercise sound professional judgment in performing job functions is essential. Effective communication skills, both verbal and written, are required. Attention to detail is crucial, along with the capability to analyze complex tasks and systems, and to design and implement effective solutions. The Clinical Pharmacist must work collaboratively and effectively with team members, patients, their families, and the public. A comprehensive understanding of the practices, principles, methods, and equipment utilized in delivering pharmaceutical services is necessary. Strong organizational and prioritization skills are also required. A current and valid California pharmacy license is mandatory.
Qualifications
Minimum Qualifications:
- At least one (1) year of experience in acute care hospital pharmacy or home infusion therapy.
- Possession of a current and valid California pharmacy license.
- Pharm.D. degree is preferred.
- Experience in I.V. admixture and unit dose distribution.
- Knowledge of pharmacokinetics dosing.
- Experience in pharmacy intervention and medication usage evaluation.
