Clinical Nurse Specialist

We are seeking a skilled Licensed Practical Nurse to join our team at the Long Island State Veterans Home. The successful candidate will have a strong background in nursing care and be able to work effectively with residents, families, and healthcare teams.
About the Role:
This is a rewarding opportunity for a compassionate and detail-oriented LPN to provide high-quality patient care in a fast-paced environment. As a member of our interdisciplinary team, you will be responsible for delivering comprehensive nursing services to residents, including assessment, planning, implementation, and evaluation of their care plans. You will also work closely with physicians, therapists, and other healthcare professionals to ensure seamless coordination of care.
Responsibilities:
* Provide direct patient care, including administering medications, wound care, and vital signs
* Develop and implement individualized care plans that meet the unique needs of each resident
* Collaborate with the interdisciplinary team to coordinate care and ensure continuity of service
* Communicate effectively with residents, families, and healthcare staff to ensure timely and accurate information exchange
* Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
* Currently licensed as a Licensed Practical Nurse (LPN) in New York State
* One year of experience working in a long-term care setting preferred
* Strong communication and interpersonal skills, with ability to work effectively in a team environment
* Ability to prioritize tasks, manage multiple responsibilities, and maintain accuracy and attention to detail in a fast-paced environment
