Position Overview:

Key Responsibilities:
  • Collaborate closely with the healthcare team to manage and implement patient care plans.
  • Conduct thorough assessments of patients and their families to tailor care appropriately.
  • Administer medications and perform clinical procedures as required.
  • Record comprehensive patient information, including medical history and current medications, into the electronic medical record (EMR).
  • Facilitate the ordering of medical supplies and maintain organized exam rooms.
  • Coordinate necessary tests, obtain prior authorizations, and manage patient referrals and appointments.
  • Engage in continuous evaluation of care processes and outcomes, providing feedback for improvements.

Required Skills and Qualifications:
  • High school diploma or equivalent is mandatory.
  • Graduation from an accredited Practical Nursing Program is essential.
  • Possession of a current LPN license is required.
  • Prior experience in a clinical setting is preferred.
  • Proficiency in electronic medical records is highly desirable, with familiarity in Athena being a plus.
  • Competence in Microsoft Office Suite, including Word, Excel, and Outlook.
