About the Position:


During a visit that can last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication adherence, social circumstances, and other potential risks.

This process offers unparalleled insight into a member's overall health and well-being, which can be challenging to capture during standard office visits.

The Matrix Comprehensive Health Assessment aims to enhance the quality of care and allows us to address multiple care gaps in a single visit.

Responsibilities:

Matrix Provider – What to Expect:
Conduct Adult/Geriatric Assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix guidelines and protocols.

Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and any relevant ancillary medical personnel as needed.

Engage with Primary Care Physicians (PCP) regarding patient education and provide necessary follow-up.

Deliver services across various settings, including Home Visits, Skilled Facility Visits, and Telehealth Visits.

Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments

Type:
PRN/Part Time - Flexible-Nurse Practitioner/Physician Assistant

PAY:
$115.00 per completed Assessment

Location:
Home, SNF, Telehealth, and other community environments

Hours:
Days, Evening, Nights, weekend, Flexible Scheduling

Benefits:
PRN/Part Time providers are non-benefited except for 401(k) and limited Voluntary Benefits.

Mileage Reimbursement plan.

Qualifications:

Must Haves:
Master's Degree required OR equivalent experience and successful completion of NP licensure.

Master's Degree required OR equivalent experience and successful completion of PA licensure.

Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.

Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.

Current BLS, ACLS, or CPR certification.

A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant. Ability to travel in-state or out-of-state.

Strong computer skills and familiarity with EMR software.

Excellent verbal and written communication skills with patients, clients, and colleagues.

A positive and outgoing personality, with the ability to be a team player. Comfortable and adaptable to frequent changes.

Travel Requirements:
Ability to travel, possess a valid state driver license, and have proof of adequate automobile insurance coverage.

Travel required to meet patients in their residences.
