Clinical Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality healthcare services to at-risk individuals. Responsibilities- Conduct comprehensive adult and geriatric assessments, medical histories, diagnoses, and treatments
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to provide coordinated care
- Develop and implement individualized care plans to address the unique needs of each patient
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Participate in quality improvement initiatives to enhance the delivery of care and patient outcomes
- Masters degree in nursing or a related field, or equivalent experience and licensure
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
As a member of our team, you will have the opportunity to travel to various locations to provide care to our patients. A valid state driver's license and adequate automobile insurance coverage are required.
