Job Overview

We are seeking a skilled Nurse Practitioner to join our team at ComForCare Home Health Care - Washington, DC. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our clients in the comfort of their own homes.

Key Responsibilities
  • Conduct Comprehensive Assessments: Perform thorough assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develop Individualized Care Plans: Create personalized care plans that incorporate analysis of data and current scientific findings.
  • Provide Interventions and Support: Deliver interventions with a focus on achieving realistic client outcomes, and provide ongoing support to clients and their families.
  • Monitor Progress and Update Care Plans: Regularly report changes and update care plans to reflect progress towards goals and outcomes.
  • Administer Medications and Maintain Records: Administer medications as needed, and maintain accurate and up-to-date client records.
  • Collaborate with Healthcare Team Members: Work closely with other healthcare team members to ensure seamless care and support for clients.
Requirements
  • Education and Licensure: Graduate of an accredited school of professional nursing, and current license as a Registered Nurse (RN).
  • Certifications and Training: Current CPR certification, and access to reliable transportation.
  • Skills and Qualities: Excellent communication and interpersonal skills, ability to maintain confidentiality, and knowledge of safety and infection control practices.
