Job Summary

We are seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team of healthcare professionals. As a Nurse Practitioner at Snowline Hospice, you will be responsible for providing comprehensive care to our patients and their families, working collaboratively with our Medical Director and other care team disciplines.

Responsibilities
  • Provide compassionate and comprehensive care to patients and their families
  • Collaborate with the Medical Director and other care team disciplines to provide support to our care programs
  • Perform bedside patient assessments and assist with diagnosis, treatment, and management of acute and chronic health conditions
  • Order, perform, and interpret laboratory and radiology tests within scope of professional practice
  • Prescribe medications, including controlled substances, to the extent delegated and licensed
  • Assist in all facets of care coordination for palliative care and hospice referrals
  • Prepare and maintain accurate patient records, charts, and documents
Requirements
  • Minimum of 2 years of experience in direct Hospice/Palliative/Geriatric/Home Based Primary Care
  • Skilled in symptom management, goals of care discussions, advance care planning, pain management, and relief
  • Palliative Care certification preferred but not required
  • Graduate of a Nurse Practitioner Program approved by the Board of Registered Nursing
  • Unrestricted Nurse Practitioner License, Current Drug Enforcement Agency (DEA) Certificate, and Furnishing number issued by the Board of Registered Nursing
  • Graduate from an accredited School of Nursing, Licensed Registered Nurse in the State of California, and Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Association - American Nurses Credentialing Center (ANCC)
