We are seeking a skilled Licensed Practical Nurse to join our team at LYFE at Schaffner Youth Shelter (Leading Youth For Excellence). The ideal candidate will have experience in nursing with a focus on child and adolescent needs.
Key Responsibilities- Ensure the availability of necessary equipment and services for residents.
- Oversee medication administration processes in accordance with regulatory requirements.
- Provide nursing administration to staff, ensuring the safe administration of medication or treatment to clients in residential services.
- Offer on-call services for Residential nursing and/or medication management services.
- Maintain up-to-date staff knowledge on client medical information.
- Ensure the timely ordering and maintenance of nursing/medical supplies.
- Develop and present medication monitoring training to direct support staff as needed.
- Prevent client abuse by adhering to policies and procedures, attending training, reporting suspicious behavior, and following mandatory abuse reporting requirements.
- Monitor chronic health conditions of residents.
- Attend meetings and participate as an interdisciplinary team member.
- Ensure compliance with regulatory, licensure, and accreditation standards concerning medication storage, dispensing, administration, and disposal.
- Monitor medication supplies and obtain refills as necessary to prevent lapses.
- Meet with residents to address medical needs.
- Provide training on infection control, first aid, CPR, medication administration, and agency medical policies and procedures.
- Train residential staff on medications, including reasons for prescribing, side effects, desired outcomes, and dosage information.
- Provide staff training on individual medical needs, such as asthma, allergies, and other conditions.
- Arrange for staff trainings on specific client health issues.
- Review staff medical documentation and medication error reports.
- Provide weekly reports to the Director on refills, dosage, and changes in prescription.
- Minimum of an Associate's Degree in Nursing (Bachelor's Degree preferred).
- Current licensure to practice in the Commonwealth of Pennsylvania.
- A valid Pennsylvania driver's license and insurance.
- Child abuse, FBI, and Pennsylvania State Police criminal background clearance.
- 4+ years of previous experience as an LPN (preferred).
- Critical thinking skills for organizing and analyzing information.
- Effective communication skills for verbal and non-verbal interactions.
- Ability to collaborate with others for optimal client outcomes.
- Demonstrated knowledge of community resources.
- Ability to complete paperwork in a timely manner.
- Excellent organizational skills.
