Job Description:

The Care Manager is responsible for providing high-quality care and services to a consistent group of residents and their families. This includes building meaningful relationships, providing assistance with activities of daily living, and notifying management of changes in condition.

Responsibilities:

  • Develop and implement Individualized Service Plans (ISPs) to meet the unique needs of each resident
  • Provide education and support to residents and their families on available resources and services
  • Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies
  • Respond to dining needs of residents and guests in a timely and courteous manner

Qualifications:

  • Dedication to serving seniors with a passion for delivering exceptional customer service
  • Ability to work in a fast-paced environment and adapt to changing situations
  • High School diploma or equivalent required; prior experience working with seniors preferred

Compensation and Benefits:

  • Competitive salary: $55,000 - $70,000 per year, depending on location and experience
