Our mission is to build enriching neighborhood health centers that are easy to access, provide or arrange for all our patients' healthcare needs, and partner with Medicare Advantage plans that fully cover primary care.
Our team is passionate about creating a new standard-bearing senior model for the United States, and we are seeking a skilled Licensed Clinical Social Worker to join our team. As a key member of our care team, you will be responsible for providing initial evaluations and individual and group therapy to patients, as well as providing care coordination and management to patients with social determinants of health challenges.
Job SummaryThe Licensed Clinical Social Worker is a critical role in our organization, responsible for providing high-quality mental health services to our patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans, and provide education and support to patients and their families.
Responsibilities- Conduct initial evaluations and assessments of patients' mental status
- Develop and implement individualized care plans, including therapeutic interventions and support services
- Provide individual, family, and group therapies based on patients' developmental and behavioral health needs
- Collaborate with physicians and other patient care team members to ensure comprehensive care
- Ensure accurate and timely documentation of patient progress and care plans
- Stay up-to-date with continuing education requirements and best practices in geriatric mental health
To be successful in this role, you will need:
- A Master's degree in social work
- Current licensure as a Licensed Clinical Social Worker with at least three years of behavioral healthcare experience
- Experience working in a primary healthcare setting, preferably with a geriatric population
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
