The Role

The Advanced Practice Clinician will be responsible for delivering and coordinating primary care services to SCAN members, aligning with the organization's mission to maintain seniors' health and independence.

Key Responsibilities

- Deliver care to an assigned group of members (patients) primarily in their residences, often within senior living facilities or skilled nursing environments.
- Conduct physical and wellness examinations, perform urgent medical visits, and provide telephonic triage to ensure timely and appropriate care, aiming to avoid hospitalizations when possible.
- Administer vaccinations, order and interpret diagnostic tests, and initiate goals of care discussions with patients and their families.
- Develop comprehensive care plans tailored to the patient, their family, and community staff.
- Collaborate with the case management team to ensure members receive timely access to necessary services.
- Independently manage a visitation schedule according to established protocols, adjusting as needed based on members' health status.
- Foster collaborative relationships with Primary Care Physicians and other healthcare professionals to promote disease prevention and health management.
- Connect with specialists, hospitals, and community organizations to ensure continuity of care and facilitate shared decision-making.
- Monitor clinical quality and address care gaps in partnership with other providers.
- Document member interactions and visits promptly in accordance with established protocols.
- Serve as a resource for members, their families, and community support staff.
- Participate in events with the Integrated Care Sales team to enhance member awareness and educate communities.
- Conduct virtual health visits as necessary to improve triaging and reduce travel time.
- Engage in an on-call program to support members after hours and on weekends.
- Maintain professional knowledge through educational workshops and literature.
- Contribute to team efforts in achieving quality healthcare goals by closing care gaps and optimizing clinical workflows.
- Actively support SCAN's Vision and Goals.

Your Qualifications
  • Master's degree in Nurse Practitioner Certificate or a Master's from an accredited PA program, with a focus on Adult/Geriatric medical care preferred.
  • A combination of education, experience, and training may be considered equivalent.
  • Current California Nurse Practitioner license or California Physician Assistant Certification (PA-C) in good standing.
  • Wound Care and Palliative Care Certificates are advantageous.
  • Knowledge of complex chronic disease management, including dementia, diabetes, heart failure, and COPD.
  • Ability to assess healthcare needs and coordinate referrals effectively.
  • Familiarity with HCC and ICD-10 coding and documentation practices.
  • Understanding of wound care and management principles.
  • Knowledge of CMS guidelines and Medicare Advantage Managed Care.
  • Comfortable initiating goals of care and end-of-life discussions.
  • Strong interpersonal skills and a proactive approach to building relationships with healthcare teams and patients.
  • Knowledge of HIPAA regulations and commitment to maintaining confidentiality.
  • Adaptable and open to problem-solving and change.
  • Basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and willingness to learn new technologies.
  • Physical ability to perform tasks such as bending, lifting, and carrying up to 50 lbs. as needed.
  • Multilingual and multicultural skills are a plus.
  • Willingness to travel 75% of the time within the assigned area; valid driver's license and reliable transportation required.
