Home Health Charge Nurse

Mass General Brigham is seeking a skilled and compassionate Charge Nurse (RN) to join our Home Hospital team. As a Charge Nurse, you will be responsible for providing high-quality patient care in the comfort of patients' homes.
Job Summary:The Charge Nurse (RN) will be responsible for coordinating patient care, managing nursing staff, and ensuring seamless transitions between care settings. This is a challenging and rewarding role that requires strong leadership skills, excellent communication, and a passion for delivering exceptional patient care.
Key Responsibilities:- Coordinate patient care, including assessment, planning, implementation, and evaluation of care plans.
- Manage and supervise nursing staff, including delegating tasks, providing feedback, and ensuring compliance with policies and procedures.
- Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication.
- Develop and maintain relationships with patients, families, and caregivers to ensure high-quality care and patient satisfaction.
- Monitor and report patient outcomes, identifying areas for improvement and implementing changes as needed.
- Current Massachusetts license as a Registered Nurse (RN).
- Minimum of one year of recent nursing experience in an acute or emergency setting.
- Home care experience preferred.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Ability to prioritize tasks, manage time, and maintain accurate records.
