About the Role

We are seeking a skilled Charge Nurse to join our Home Hospital team at Mass General Brigham. As a Charge Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients and their families to promote health and wellness.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Manage patient schedules and adjust as needed to meet clinical care and program requirements.
  • Document patient information and changes in status/plan of care in a timely and accurate manner.
  • Reconcile medications accurately and timely at admission, discharge, and transfer.
  • Make referrals to other disciplines and community resources as needed to maximize patient safety and outcomes.
Requirements
  • Current Massachusetts license as Registered Nurse.
  • Minimum of one year recent nursing experience in acute or emergency setting required.
  • Home Care experience a plus.
  • Ability to work with various computer software applications and technologies required.
  • Strong interpersonal and customer service skills and ability to work with a diverse population.
  • Ability to communicate effectively in writing, verbally, and electronically.
  • Ability to work as a member of an interdisciplinary team of healthcare providers.
  • Ability to organize and prioritize work.
  • Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
