About the Role:
- Develop and implement individualized physical therapy treatment plans for patients in their homes.
- Conduct thorough assessments to identify patients' physical strengths and limitations, and create personalized rehabilitation programs.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care and make informed treatment decisions.
- Maintain accurate and detailed records of patient progress and treatment outcomes.
Requirements:
- Master's degree from an accredited Physical Therapy Program.
- Minimum of one year of experience in physical therapy, preferably in a home health setting.
- Current and unrestricted Physical Therapy license and CPR certification.
- Excellent organizational and communication skills, with the ability to work independently and as part of a team.
