The primary role of the Rehabilitation Therapy Assistant (RTA) is to deliver direct therapeutic interventions as outlined, assigned, and overseen by the Physical Therapist. This position requires meticulous documentation of treatments administered and tracking of patient progress in medical records. You will be entrusted with ensuring that the highest standards of care are consistently provided to our residents.

KEY RESPONSIBILITIES:

Resident Care: 65% of the role

  • Implement individualized therapy plans and necessary modifications to enhance client independence and functionality under the guidance of a Licensed Physical Therapist and in accordance with physician directives.
  • Exhibit proficient technical skills aligned with current standards of practice.
  • Communicate any changes in the resident's condition effectively.
  • Engage in discharge planning and community mobility initiatives.
  • Recommend essential durable medical equipment to promote independence.
  • Ensure accurate and timely billing records and clinical documentation.
  • Address family inquiries and concerns promptly.
  • Prevent any instances of resident abuse or neglect.
  • Maintain confidentiality and security of all patient information.

Training and Education: 20% of the role

  • Facilitate and provide training alongside Restorative Nursing and other Care staff.
  • Deliver educational sessions to patients, caregivers, and community partners.
  • Assist in the training and development of Bluestem Village staff and professional partners.
  • Ensure adherence to all community-required education, training, and competency programs.
  • Participate in PTA student development and educational initiatives.
  • Contribute to collaborative care meetings.

Department Responsibilities: 15% of the role

  • Conduct screening rounds as directed by the Director of Rehabilitation to evaluate client needs and safety in the environment.
  • Provide insights to the Director of Rehabilitation and team members regarding opportunities for operational improvements, safety issues, or other updates.
  • Assist in maintaining cleanliness and readiness of the therapy department.
  • Perform additional duties as assigned.

SKILLS AND KNOWLEDGE:

Knowledge: Familiarity with current Colorado physical therapy practices, regulations, and guidelines relevant to nursing care facilities.

Skills: Experience in providing care within post-acute or long-term care settings (preferred).


EDUCATION AND EXPERIENCE:

Education: Must have graduated from an accredited two-year college program recognized by the American Physical Therapy Association.

Experience: Background in delivering care in post-acute or long-term care environments (preferred).

License: Must hold a current, unencumbered, active Colorado Physical Therapy Assistant License.

Certifications: CPR certification as required.


PHYSICAL DEMANDS:

  • Frequent squatting, twisting, and reaching.
  • Regular walking and standing.
  • Occasional handling, transferring, and lifting of residents, exceeding 100 lbs.
  • Occasional crawling, climbing, and sitting.
  • Frequent repetitive pushing, pulling, and grasping up to 50 lbs.

  • Must possess a valid, unrestricted Colorado driver's license.
