Physical Therapist Assistant

We are seeking a highly skilled Physical Therapist Assistant to join our team at Bluestem Village. As a key member of our rehabilitation department, you will play a vital role in providing exceptional care to our residents.
Key Responsibilities- Villager Care: 65% time
- Develop and implement individualized treatment plans to enhance villager independence and functioning, under the supervision of a licensed physical therapist and in accordance with physician orders.
- Provide competent technical skills in accordance with current standards of practice.
- Communicate changes in villager condition to ensure seamless care.
- Participate in discharge and community mobility planning to ensure a smooth transition.
- Prescribe necessary durable medical equipment to facilitate independence.
- Maintain accurate and timely billing records and supporting clinical documentation.
- Respond to family requests and concerns in a timely and professional manner.
- Ensure villager safety and prevent abuse or neglect.
- Protect confidentiality and safeguard all patient information.
- Training/Education: 20% time
- Coordinate and provide training with restorative nursing and other care staff.
- Deliver education and training to patients, caregivers, and community partners.
- Assist with training and education of Bluestem Village employees and professional partners.
- Ensure compliance with all community mandatory education, training, and competence programs.
- Participate in PTA student development and educational programs.
- Collaborate with care teams during conferences.
- Department Duties: 15% time
- Perform screening rounds as directed by the Director of Rehabilitation to assess villager needs and environmental safety.
- Provide recommendations to the Director of Rehab and other team members on opportunities for business improvement, safety concerns, or updates.
- Maintain the cleanliness and tour readiness of the therapy department.
- Perform other duties as assigned.
- Graduate of an accredited 2-year college-level program approved by the American Physical Therapy Association.
- Current, unencumbered, active Colorado Physical Therapy Assistant License.
- Experience in post-acute or long-term care environment (preferred).
- CPR certification (as requested).
- Frequent squatting, twisting, and reaching.
- Frequent walking and standing.
- Occasional villager handling, transferring, and lifting from floor, over 100#.
- Occasional crawling, climbing, and sitting.
- Frequent repetitive pushing, pulling, and grasping up to 50#.
- Maintain a valid, unrestricted Colorado driver's license.
