We are seeking a passionate Registered Nurse Case Coordinator to contribute to our mission.
Role Responsibilities and QualificationsThe Case Coordinator is responsible for delivering exceptional, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. This role involves collaboration with the Rehabilitation Program Director and the Facility Case Management Director to ensure seamless departmental operations.
Key responsibilities include:
- Conducting program orientations for patients and their families, detailing the case manager's role, rehabilitation philosophy, and criteria for continued care and discharge.
- Coordinating educational efforts for patients and their families, fostering their involvement in discussions about care plans and progress.
- Implementing individualized treatment plans that reflect the patient's strengths, needs, and preferences while promoting active participation throughout the rehabilitation journey.
- Documenting findings from discharge planning evaluations and psychosocial assessments, communicating any needs or preferences of the patient and their family.
- Upholding a commitment to exceptional patient interactions that enhance the quality of service provided.
- Current RN licensure in the state; a BSN is preferred.
- A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
- Preferred certifications in case management, nursing, or utilization review.
- Familiarity with InterQual guidelines is advantageous.
- Basic Cardiac Life Support certification must be obtained within 30 days of employment.
- An Associate Degree or a diploma in nursing is required.
