About the Role

We are seeking a highly skilled Licensed Couples Family Therapist to join our team at Sunbelt Healthcare LLC. As a key member of our interdisciplinary team, you will provide therapeutic services for members of our organization, utilizing your expertise in behavioral health and chronic health conditions.

Key Responsibilities
  • Coordinate care for clients with dual diagnoses, substance use, general mental health, serious mental illness, and physical health conditions.
  • Provide a variety of outpatient therapy services to clients with mental illness, addiction disorders, trauma effects, and chronic conditions.
  • Work with other team members to provide a continuum of services designed to aid in the client's recovery goals and enhance outcomes.
  • Manage the coordination of care of clients to help develop service plans, integrated assessments, and recovery objectives.
  • Maintain confidentiality of records aligning with HIPAA requirements, assess for risk of harm to self and others, and complete initial and ongoing assessments of client's assigned to your panel.
Requirements
  • Master's degree in counseling, social work, or related field.
  • Active License in the state of Arizona.
  • Independently Licensed clinician (highly preferred).
  • Bilingual (preferred).
  • Knowledge of Microsoft Software especially 365 platform.
  • Experience with electronic health records (EHR).
  • Strong organizational skills and communication skills.
  • Knowledge of integrated healthcare delivery models.
