Job Summary

We are seeking an experienced RN-Case Manager to join our SSM Health float pool team. This individual will float to our 6 St. Louis locations, providing exceptional care to patients and ensuring a smooth transition to their next level of care.

Key Responsibilities
  • Assess patients' discharge planning needs and develop a plan to ensure a safe and effective transition to their next level of care.
  • Identify and escalate gaps/barriers related to inpatient and observation length of stay.
  • Participate in the utilization management process, ED case management process, admission process, and act as a case manager liaison with post-acute providers.
  • Apply evidence-based practice and scientific knowledge to nursing practice, ensuring that nursing care is delivered based on patients' age-specific needs and clinical needs.
Requirements
  • Graduate of an accredited school of nursing or education equivalency for licensing.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
