Job Summary

BrightStar Care of Sugar Land is seeking a skilled Licensed Vocational Nurse (LVN) to join our team as an Intake Coordinator. As an LVN, you will play a vital role in our intake process, working closely with our team to ensure seamless patient care.

Responsibilities
  • Conduct thorough patient assessments and develop individualized care plans
  • Collaborate with our skilled care team to ensure timely and effective patient care
  • Manage patient intake and onboarding process, ensuring all necessary documentation is completed
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
  • Participate in ongoing quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Current unrestricted LVN license in the state of Texas
  • 1+ years of experience as an LVN, with a focus on home care or skilled care
  • Reliable transportation and a valid driver's license
  • Current CPR/BLS certification
  • Negative TB test
