Key Responsibilities- Conduct thorough physical health assessments, including HIV risk assessments, general first aid, and triage activities.
- Screen for substance abuse, developmental health, mental health, and physical body checks for signs of abuse.
- Manage and train clients regarding chronic conditions, such as asthma and diabetes.
- Take vital signs, heights, and weights, and document observations and activities.
- Provide immunizations and document same.
- Administer first aid and emergency care as needed.
- Execute medical orders, including tests, tasks, and hospitalizations, as ordered by a physician or nurse practitioner.
- Obtain lab specimens and administer hearing and vision tests.
- Provide training and health education related to sexual reproductive health.
- Administer medications and manage medication reconciliation.
- Ensure required documentation for all case actions is completed within established timeframes.
- Maintain professional boundaries with clients and staff.
- Maintain a safe and secure medical environment for clients.
- Current Licensed Practical Nurse in New York State.
- Current CPR certification.
- Completion of required courses in Child Abuse and Infection Control.
- Valid Driver's License with a satisfactory driving record.
