Key Responsibilities
  • Deliver high-quality patient care under the guidance of our registered nurses and according to individualized care plans.
  • Assist patients with self-administered medications, ensuring their safety and well-being.
  • Monitor and report vital signs, including temperature, pulse, and blood pressure, to our healthcare team.
  • Support patients with daily living activities, such as bathing, grooming, and skin care.
  • Assist with feeding patients, promoting healthy nutrition and hydration.
  • Provide mobility assistance, exercises, and positioning to ensure patient comfort and safety.
  • Document patient interactions, care rendered, and any changes in their condition to ensure seamless care coordination.
  • Practice infection control principles to maintain a clean and safe environment.
Requirements
  • High school diploma or equivalent
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months
  • Meet applicable state certification requirements
  • Valid driver's license, auto insurance, and reliable transportation
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency
