Skilled Nursing Facility Registered Nurse

Job Summary:
At BridgePoint Healthcare, we are dedicated to providing patient-centered, individualized care for patients requiring longer hospitalizations in post-acute care settings. As a Registered Nurse - SNF, you will play a vital role in delivering high-quality care to our patients.
Responsibilities:
- Develop and implement individualized care plans in coordination with residents, families, and interdisciplinary staff.
- Provide direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.
- Communicate changes in resident's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate.
- Participate in the discharge planning process to ensure a smooth transition for our patients.
Requirements:
- Graduate of an accredited Bachelor of Science in Nursing or Associate Degree in Nursing program.
- Current RN - Registered Nurse license from the District of Columbia Department of Health required.
- ACLS & BLS certification required.
- Minimum one (1) year's recent nursing experience preferred.
