About the Job

This role as an Independent Living Coordinator at Clarvida involves working with children, youth, and families in conjunction with Children & Youth Services. You will initiate and maintain contact with youth who are referred through CYS and conduct approved assessments to develop Independent Living Goal plans within the first month of contact.

The key responsibilities of this position include:

  1. Conducting quarterly reviews of Independent Living plans with each youth to ensure their needs are being met.
  2. Scheduling, planning, and completing group activities based on the participating youth's needs and availability.
  3. Preparing and completing required documentation, including case notes, in compliance with regulations and company standards.
  4. Being available and prepared to provide court testimony in a professional manner as needed and scheduled.

Requirements

To be considered for this role, you must have:

  • A Bachelor's Degree in a related field, or a Bachelor's Degree in an unrelated field with a minimum of 12 credits in a Human Services-related course.
  • High School Diploma with 5 years of experience in Human Services.
  • A valid Pennsylvania Driver's license with a good driving record.
