Job Description
As a compassionate care provider for skilled nursing facilities, you will have the opportunity to make a meaningful difference in the lives of our patients. This role involves conducting comprehensive assessments, developing personalized care plans, and working collaboratively with interdisciplinary teams to ensure the best possible outcomes for our residents.
Responsibilities:
- Conduct thorough assessments of patients, including physical exams, medication reconciliation, and social determinants of health evaluation.
- Develop and communicate problem lists and recommended treatment plans for each patient.
- Ensure accurate and complete documentation, including ICD-10 coding.
- Collaborate effectively with the interdisciplinary care team to ensure seamless care coordination.
- Facilitate advanced illness and advanced care planning conversations with patients and their families.
Requirements:
- Active NP license in the state of practice, free of any disciplinary actions.
- Minimum 1 year of NP experience.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
- Reliable transportation for assessments in nursing homes, assisted living, and home settings.
- DEA certification.
