As a Support Professional, you will play a vital role in providing in-home and in-community support to individuals with physical disabilities, brain injuries, and mental health disorders.

Key Responsibilities
  • Support Individuals in Learning and Independence: You will support individuals in learning, maintaining, and improving skills through their participation in a variety of activities of everyday life, including in their homes and in the community to improve their level of independence.
  • Implement Support Strategies: You will implement support strategies to achieve the individual's personalized goals, working closely with family members and existing providers to ensure a comprehensive approach.
  • Document Progress: You will submit accurate and complete daily progress notes for each shift worked with the individuals, providing valuable insights into their progress and development.
Requirements
  • Education and Experience: A Bachelor's Degree or at least 2 years comparable, community-based, life or work experience providing services to individuals with disabilities.
  • Valid Driver's License: A valid driver's license and existing insurance are required, as you will be driving individuals in personal cars throughout the community.
  • Flexibility and Availability: You must be willing to drive individuals in personal cars throughout the community and have flexible availability to meet the needs of our clients.
  • Excellent Communication Skills: Excellent organizational, interpersonal, and communication skills are essential for this role, as you will be working closely with individuals, family members, and existing providers.
