Position Overview:
As a Nutrition Care Intern, you will play a vital role in delivering nutritional support and education to participants at PACE of Southwest Michigan.

Under the guidance of the Center Manager or designated supervisor, you will utilize your advanced knowledge and skills in dietetics independently.

Key Responsibilities:
  • Assess and evaluate the nutritional status of participants.
  • Develop and implement personalized nutritional care plans.
  • Determine nutrient requirements and recommend dietary prescriptions.
  • Monitor and evaluate the effectiveness of nutritional interventions.
  • Communicate nutritional information effectively with healthcare professionals.
Qualifications:
Applicants should possess a Bachelor's Degree from an ADA-accredited institution with a major in nutrition.
Certification as a Certified Diabetes Educator (CDE) is preferred. A National Provider Identifier (NPI) number is required to enhance operational efficiency and mitigate fraud.

Training and Compliance:
Interns must adhere to a standardized set of competencies established by PACE of Southwest Michigan and approved by CMS prior to independent practice.

Work Environment Considerations:
Interns may encounter exposure to infectious materials, diseases, and hazardous substances.

Patient Demographics:
Care is provided primarily to adult and geriatric populations.

Education and Community Engagement:
Provide nutritional education to participants and caregivers as directed by physicians, in alignment with ADA standards.
  • Engage in community nutrition education initiatives.
  • Coordinate meal delivery with the Food Service Provider.
  • Document participant progress accurately in medical records and communicate changes to the healthcare team.
  • Assist in monitoring meal intake, diet adherence, and tray accuracy.
  • Conduct inspections to ensure food preparation and kitchen sanitation.
  • Oversee kitchen operations to uphold food safety standards.
  • Monitor daily meal counts to ensure accurate meal delivery.
  • Collaborate with the Food Service Provider to ensure meals meet nutritional needs.
  • Contribute to the development of nutritional policies at PACE of Southwest Michigan.
  • Participate actively in the Quality Improvement Process (QIP).
  • Comply with all PACE of Southwest Michigan policies and regulations.
  • Exhibit flexibility in work scheduling.
  • Demonstrate understanding of the physical, mental, and social needs of older adults.
  • Possess effective skills in physical assessment and chronic disease management.
  • Utilize computer skills proficiently in the workplace.
