Primary Care Specialist Opportunity

Bluestone Physician Services is seeking a skilled Nurse Practitioner or Physician Assistant to provide exceptional team-based care to patients residing in Assisted Living and Memory Care communities. As a Primary Care Specialist, you will be responsible for delivering high-quality, patient-centered care that addresses the unique needs of our patients.

Key Responsibilities:

  • Provide on-site primary care to patients within Assisted Living and Memory Care communities
  • Manage a panel of 200 patients, with the ability to design and manage your schedule
  • Develop and implement care plans that meet the needs of patients and their families
  • Collaborate with clinical support teams, including LPNs, RNs, and Social Workers
  • Access diagnostics, such as labs and x-rays, as needed
  • Collaborate with community partners to provide comprehensive care

Requirements:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
  • Board Certified or Board Prepared through AANP, ANCC, or NCCPA
  • Licensed or able to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
  • Ability to obtain and/or maintain DEA certification for the State of Minnesota
  • Proven ability to communicate effectively and possess excellent interpersonal and customer service skills
