The Registered Nurse Case Manager will collaborate with the Enterprise Director of Nursing and the Baraboo Branch Manager to ensure high standards of care for our clients. Responsibilities will include conducting initial client assessments, supervising the care of current clients (both personal and skilled), and coordinating care with skilled service organizations and hospices. Key attributes for success in this role include compassion, patience, and an engaging personality to foster trust and comfort with clients and their families.

Essential Duties and Responsibilities:

  • Deliver care in accordance with the state's nurse practice act
  • Conduct initial evaluations and regularly reassess clients' nursing and personal assistance needs
  • Implement preventive and rehabilitative nursing procedures while advising clients and families on related needs
  • Adhere to agency policies and procedures
  • Evaluate staff competency to ensure quality, continuity, and safety of client care
  • Encourage home care referrals within the healthcare community

Qualifications:

  • Current, unencumbered Registered Nurse license in Wisconsin
  • A minimum of three years of documented nursing experience is required, with two years of clinical experience and one year in a home health agency or case management preferred
  • Knowledge of regulatory compliance, including local, state, and federal laws, Joint Commission standards, OSHA, and HIPAA
  • Strong time management skills and flexibility are essential
  • Willingness to travel locally
