About the Role

We're seeking an experienced Senior Medical Assistant to join our team in Perimeter, GA. In this role, you'll provide critical support activities to coordinate personalized care delivery for Upperline Plus patients and their families. You'll work closely with our clinicians to identify and coordinate the needs of patients with multiple chronic conditions, ensuring they receive the best possible care.

Responsibilities

* Conduct health assessments and complete visits with Upperline Plus patients in our Podiatry Clinics
* Complete vitals and required documentation in our EMR system (AthenaHealth)
* Provide routine foot care for at-risk patients with Diabetes and/or other chronic illness
* Facilitate and support virtual provider encounters in the clinic
* Telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care
* Care coordination activities as assigned by our Nurse Practitioner, including medical records requests, order management, appointment scheduling, and managing patient care referrals
* Visit patients in their homes to complete routine foot exams as needed
* Identify community resources and assist our patients and families in accessing those resources
* Participate in weekly interdisciplinary team meetings

Qualifications

* A minimum of 2 years' experience as a medical assistant providing direct patient care is required
* Experience in an outpatient clinic setting is strongly preferred
* Clinical care coordination or care management experience is ideal
* Previous experience assisting or providing care to the adult-geriatric patient population is preferred
* Prior wound care and foot care experience is a plus
* Bilingual English/Spanish is helpful
* Previous experience working in an EMR system, ideally AthenaHealth
* Database management skills and proficiency using Word, Excel, and other Microsoft Office products is necessary
* Must have reliable transportation, a valid driver's license, and minimum state required liability auto insurance
* Extremely strong customer-service orientation and work ethic
* Exceptional organizational skills and ability to multi-task
* Self-starter with an entrepreneurial spirit and demonstrated problem-solving skills
* Excellent oral and written communication skills to ensure effective interactions with patients, family caregivers, clinicians, and team members
