Job Description

We are seeking an experienced Licensed Practical/Vocational Nurse to join our dedicated team of healthcare professionals. In this critical role, you will be responsible for providing direct patient care, administering medications, and collaborating with our interdisciplinary team to develop and implement individualized care plans. Responsibilities

- Provide direct patient care, including vital sign monitoring, medication administration, and wound care
- Develop and implement individualized care plans in collaboration with our interdisciplinary team
- Collaborate with patients, families, and caregivers to ensure seamless transitions and effective communication
- Maintain accurate and up-to-date medical records, adhering to HIPAA guidelines and regulatory requirements

Requirements

- Current LPN/LVN license
- Experience working in a behavioral health or long-term care setting
- Strong understanding of medication administration, wound care, and vital sign monitoring
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders
