Job Summary

We are seeking a Caregiver/Medication Technician to join our team at LakeHouse New Holstein. This role involves providing hands-on care, physical and emotional support to each resident while maintaining a safe and comfortable home-like environment.

Responsibilities
  • Provide care and support to a designated group of residents during the shift, ensuring their physical and emotional needs are met.
  • Observe, report, and document any changes in resident condition, including symptoms, behavior, and physical changes.
  • Respond promptly to security system and resident call bells, and notify supervisor and/or Health Care Coordinator of any increased care needs.
  • Assist with continence management, serve and collect food trays, and maintain cleanliness of resident suites and community restrooms.
  • Practice good standard care precautions, including cleanliness, hygiene, and health.
  • Help residents maintain independence, promote dignity, and physical safety.
  • Assist with activities of daily living, such as bathing, dressing, toileting, grooming, ambulation, transferring, and eating.
  • Engage residents in life skills and other life enrichment activities.
  • Provide excellent customer service to internal and external customers, guests, family members, residents, vendors, and team members.
  • Perform resident laundry as assigned and needed.
Medication Technician Duties
  • Review service plans to learn pertinent information about residents.
  • Assist and observe medications and treatments for each resident, using the medication observation record and the Six Rights of Medication Pass.
  • Document and initial forms as medications are given, including documentation for refusal or missed doses.
  • Maintain confidentiality of all resident information, including resident medication.
  • Report any resident concerns made while assisting with medication to the Nurse and/or Health Care Coordinator.
  • Restock medication cart after all medication passes.
  • Assist in checking medication, regardless of packaging system.
  • Count all narcotics with another Medication Care Manager or Nurse each shift.
  • Maintain and clean the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications, and expired medications.
  • Follow the re-fill process for medications.
  • Participate in the development of the Service Plan and monthly updates.
  • Take and record temperature, blood pressure, weight, pulse, and respiration rates.
  • Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
Qualifications
  • Certified Nurse's Aid certification preferred.
  • High School diploma/GED.
  • Must be 18 years of age.
  • CPR Certification preferred.
  • First Aid Certification preferred.
  • Previous experience working with seniors preferred.
