We're seeking a Clinical Pharmacist to join our Practice Extend Team, where you'll work alongside our Care Team to improve patient health outcomes through medication therapy management.

**Job Description**

This role requires a strong background in medication therapy management, transitions of care, and population health management. As a Clinical Pharmacist, you'll conduct Comprehensive Medication Reviews, Medication Reconciliation Post-Discharge, and provide support to patients and providers through various channels.

**Required Skills and Qualifications**

  • Graduate of an accredited School of Pharmacy
  • Doctor of Pharmacy (Ph.D.) degree
  • Current Pharmacist licensure in TX, CO, AZ, NM, or WY, in good standing
  • Ability to obtain additional state licensures based upon business need
  • 3+ years of direct patient care experience conducting Medication Therapy Management and Comprehensive Medication Reviews in a clinical setting
