As a Clinical Pharmacist at Cedars-Sinai Angeles Clinic, your role will involve delivering expert medication management and addressing inquiries related to pharmaceuticals from healthcare professionals.
- Identify, address, and avert potential medication-related issues, including unaddressed indications, inappropriate drug choices, suboptimal dosages, and adverse drug reactions.
- Educate patients and healthcare providers on medication usage, potential side effects, and drug interactions as necessary.
- Collaborate with physicians and pharmacists to suggest modifications in medication therapy to enhance patient outcomes.
- Assist in the completion of medication prior authorizations as required.
- Document patient progress and outcomes for future reference and reporting.
- Ensure continuous improvement in the pharmaceutical care services offered.
- Provide educational presentations to physicians and contribute to internal newsletters.
- Mentor pharmacy residents and students during their rotations.
- Engage in departmental meetings and discussions.
QualificationsEducational Background:
- Doctor of Pharmacy (PharmD)
- California Pharmacist License
- Basic Life Support certification from an accredited organization
- Experience with USP 795 & 800 guidelines
- Familiarity with EPIC electronic health records
- Oncology-related experience is highly desirable
- Ability to prioritize and manage multiple tasks in a fast-paced environment
- Strong written and verbal communication skills in English
- Proficiency in Microsoft Office Suite (Outlook, PowerPoint, Excel, etc.)
- Ability to perform various physical activities including lifting, standing, walking, and sitting. Must possess the physical capability to fulfill all patient care responsibilities in a demanding work environment.
