Job Summary

We are seeking a skilled RN Case Manager to join our team at Trinity Health. As a key member of our care management team, you will play a vital role in ensuring the highest quality patient care and outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' holistic health status and needs, including clinical condition, support systems, and community resources.
  • Screen patients for identified and potential needs, making referrals to appropriate multi-disciplinary services as needed.
  • Develop and implement case management plans, including assessments, client/family participation, and discharge plans of care.
  • Educate staff, clients, and medical communities on care coordination and regulatory issues that impact care and outcomes.
  • Document clearly and timely in the Electronic Health Record and other required systems to ensure effective communication along the continuum.
  • Collaborate with Utilization Management staff regarding payer information, level of care, and status determinations for patients.
  • Identify opportunities for improvement and actively work with healthcare and facility teams to correct or improve results.
  • Develop and utilize tools to effectively evaluate, summarize, and communicate patients' progress toward outcomes to members of the healthcare team and patients and families.
  • Have a working knowledge of financial and reimbursement processes, including Medicare DRG payment, Case Rates, and Managed Care Risk Contracting.
  • Coordinate and conduct patient care rounds and family conferences.
  • Collaborate with Unit Director in strategies/tactics to achieve unit-based goals, such as quality, financial, and safety indicators.
  • Demonstrate a wide theory/research base that supports sound clinical and psychological orientation, including knowledge of pathophysiology, natural history of disease processes, and impact on functional ability and level of care.
  • Serve as a professional role model/mentor and change agent to develop and assist others.
Requirements
  • Possession of a current or compact state license as a Registered Nurse issued/defined by the State of Iowa.
  • Three years RN or Care Coordination experience, required.
  • Bachelor's degree in nursing or actively pursuing (at least six credits per year). A Bachelor's degree in a related field is also acceptable.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
