We are seeking a highly skilled Registered Nurse Case Manager to join our team at Enhabit Home Health & Hospice. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating with healthcare professionals to ensure seamless care transitions.
Responsibilities- Perform home visits and complete coordination of client care
- Oversee the frequency of visits for each episode
- Consult with physicians and office staff as needed to ensure accurate patient care
- Collect and analyze OASIS data to inform care decisions
- Ensure medication profiles are up-to-date and lab values are reported to physicians in a timely manner
- Attend weekly case conferences and monthly case manager meetings to stay informed and share best practices
- Current licensure as a Registered Nurse (RN) in the state of employment
- Minimum of one year of clinical experience preferred
- Demonstrate knowledge and skill in current nursing practice
- Valid state driver's license and automobile liability insurance
