Patient Intake Specialist

The LPN Admissions Coordinator is tasked with overseeing all incoming referrals to the organization, ensuring compliance with both internal policies and external regulations.
Patient Management:
- Maintains a thorough understanding of current home health coverage standards, admission criteria, documentation necessities, coding protocols, and care planning through case discussions; manages patient care accordingly.
- Effectively conducts initial home visits, introducing services, clarifying admission criteria, and guiding the process for confirming patient eligibility while obtaining necessary consents.
- Evaluates the patient and caregiver's readiness and capacity to learn care techniques, documenting their responses to educational efforts.
- Develops and oversees the aide care plan, adjusting it based on patient progress and ensuring regular evaluations of home health aide care as per state requirements.
- Supervises the Certified Nursing Assistant's involvement in the patient's care plan and skilled interventions as dictated by state regulations.
- Initiates the care plan and associated nursing actions; conducts goal-oriented visits to ensure continuity of care among nursing team members.
- Provides education to patients and families in accordance with the plan of care, assessing and documenting their responses.
- Acts as an advocate for the patient as necessary.
- Completes a comprehensive initial assessment, including OASIS and other evaluations to identify home care needs and patient history.
- Utilizes health assessment data and input from the healthcare team to determine patient requirements.
- Effectively manages expectations of patients and families regarding agency services and discharge goals.
- Establishes primary and secondary diagnoses based on thorough patient assessments.
- Formulates a care plan that includes skilled interventions and necessary medical supplies to achieve desired outcomes.
- Ensures that home health visits and medical supplies align with planned interventions and discharge objectives.
- Regularly reviews patient progress in collaboration with team members, adjusting the plan of care as needed.
- Conducts ongoing OASIS assessments and revises the plan of care accordingly.
- Identifies discharge planning needs during care plan development and implements community referrals prior to discharge.
Coordination Responsibilities:
- Prepares clinical documentation and notes within required timelines.
- Obtains physician orders for treatment modifications and communicates changes to relevant team members.
- Tracks assigned cases and organizes schedules to meet individual patient needs as outlined in their plan of care.
- Meets productivity standards set by the agency.
- Requests time off in accordance with agency policies.
- Communicates with the Clinical Supervisor regarding care plan coordination and staffing needs.
- Ensures availability of necessary equipment and supplies to support care plans, documenting usage per agency policy.
- Provides guidance to other team members.
- Updates primary physicians as necessary and at least every sixty days.
- Facilitates ongoing discussions regarding patient goals and care needs, adjusting plans to enhance progress toward discharge.
- Plans and coordinates clinical staff assignments with input from management and physicians.
- Collaborates with staff to ensure effective patient care services.
- Acts as a liaison between clinical staff and community healthcare providers, communicating changes in patient status.
- Evaluates potential referrals by reviewing facility documentation.
- Gains knowledge of home care level of care issues and insurance reimbursement processes.
- Participates in case conferences and training sessions as needed.
- Coordinates with agency staffing specialists to ensure adequate coverage for new referrals.
- Works with RN Case Managers to facilitate efficient admissions of new referrals.
- Ensures timely completion of documentation and data entry for new intakes, transferring essential medical information to relevant parties.
- Maintains accurate client medical data throughout the intake process.
- Informs management of any proposed changes affecting the intake process.
- Addresses client complaints appropriately.
- Attends meetings to ensure consistent communication regarding new intakes and existing cases.
- Supervises Team Coordinators to ensure effective management of client schedules.
- Ensures that intake processes comply with local, state, and federal regulations as well as agency policies.
- Oversees the recertification process every sixty days, ensuring timely completion.
- Reviews medical records and updates treatment plans as necessary.
- Conducts audits of medical records during recertification and forwards findings to the Director of Patient Care Services.
- Checks recertification treatment plan summaries for errors before submission for physician approval.
- Correlates audits with OASIS and other quality assessments.
- Assists with billing audits as required.
- Communicates effectively to gather patient information for services.
- Builds relationships with hospital and insurance case managers to ensure quality care.
- Ensures all clinical information is provided to insurance companies for service authorization.
- Maintains a dashboard for pending referrals requiring authorization.
- Uploads authorization documents into patient electronic charts.
- Enters authorization data into electronic systems.
- Participates in team discussions regarding patients needing authorization.
- Maintains confidentiality of patient and company information.
- Provides timely notification of absences or tardiness without misuse.
Additional Responsibilities:
- Engages in personal and professional development, maintaining current licensure and seeking learning opportunities.
- Contributes to the Quality Assurance and Performance Improvement (QAPI) program.
- Attends all required training sessions and in-service programs.
