Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate total plan of care with other healthcare professionals involved in care and ensure continuity of patient care.
- Develop and implement individualized plans of care, ensuring patients meet home health eligibility and medical necessity guidelines.
- Conduct initial and comprehensive nursing evaluations, determining primary focus of care and submitting accurate and timely documentation.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
