Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Compassus team. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with complex and chronic conditions.
Responsibilities- Assess patients' physical and emotional needs, developing and implementing individualized care plans to promote optimal comfort and quality of life.
- Collaborate with healthcare teams to coordinate care, ensuring seamless transitions and effective communication.
- Provide education and support to patients and families, empowering them to make informed decisions about their care.
- Conduct routine and emergency assessments, recording observations, treatments, and other pertinent information.
- Communicate with Medical Directors, Attending Physicians, and other healthcare professionals to ensure comprehensive care.
- Supervise nurse trainees, including graduate and advanced practice nurses, in field placements.
- Master's degree in Nursing required.
- Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
- Active NPI number.
- Maintain active DEA certification if state allows prescription of controlled substances by APPs.
This role requires regular standing, walking, and manual dexterity, as well as the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
