Key Responsibilities- Assessment and Planning: Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
- Patient Interview: Perform focused interviews to identify specific patient needs and develop individualized care plans.
- Signs and Symptoms: Assess and interpret signs and symptoms indicating physiologic and psychological changes in the patient's condition.
- Data Collection: Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
- Plan of Care: Develop and implement a plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
- Action Prioritization: Identify and prioritize actions based on patient care requirements.
- Consistency and Effectiveness: Monitor consistency between the patient plan of care and actions of medical staff, and evaluate the effectiveness of nursing interventions.
- Safe Environment: Provide a safe, comfortable, and therapeutic patient environment.
- Patient Education: Initiate and evaluate patient and/or family education.
- Plan Revision: Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
- Documentation: Document patient's response to nursing and other interventions.
- Clinical Judgment: Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities.
- Environment and Equipment: Monitor safety and effectiveness of the environment and equipment.
- Interpersonal Communication: Participate in activities to facilitate good interpersonal communications with the chosen facility.
- Licensure: Current Licensed Practical Nurse (LPN) License for the state in which the nurse practices.
- Language: Full command, verbal and written, of the English language.
- Health Certificate: Current Health Certificate (if applicable).
- PPD or Chest X-Ray: Current PPD or Chest X-Ray.
- Experience: One year prior Long Term Care (LTC) Licensed Practical Nurse (LPN) experience preferred.
- BLS: Current BLS card.
- Screening Tests: Successful completion of screening tests for the clinical specialty area(s) in which the Long Term Care (LTC) Licensed Practical Nurse (LPN) practices.
