Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work in post-acute facilities, providing high-quality patient care and making a meaningful difference in the lives of our residents.

Key Responsibilities
  • Provide direct patient care, including tracking vital signs, administering medications, and monitoring health concerns
  • Conduct intakes, document health concerns and medical history, and review residents' charts and medical records
  • Assist with the cultivation of a safe and comfortable facility environment
  • Maintain accurate records and reports, including medication administration and patient progress
  • Participate in IV set up, immunizations, and phlebotomy procedures, as needed
Requirements
  • Current Maryland state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High School diploma or equivalent
  • Minimum of 1 year of experience as an LPN or in a related role
