We are seeking a highly skilled and compassionate Certified Practical Nurse to join our team at Cobble Hill LifeCare, a leading long-term care facility in the heart of New York. As an LPN on our team, you will play a vital role in providing exceptional patient care and contributing to our mission of excellence in residential and community care.

Estimated Salary: $70,000 - $90,000 per year

About the Position:

The LPN functions under the direction and supervision of the RN/charge nurse and the Nursing Supervisor. Responsibilities include but are not limited to the following:

  • Coordinating the resident's care in collaboration with the CNAs, physician, and other disciplines through assessment, under the guidance of the RN/Supervisor, and involvement with the resident and family.
  • Leading and communicating with the team in the planning and implementation of care and thorough documentation of the same in the EMR.
  • Providing proficient wound care, ostomy care, foley catheter insertion and care, medication administration through all routes including G-tube and IVs, trach care and suctioning.

Requirements:

To be successful as an LPN at Cobble Hill LifeCare, you should have:

  • A graduate degree from an approved school of nursing
  • An active LPN license in the State of New York.
  • CPR certification
  • The ability to observe and identify the resident's needs, report to the RN/Supervisor and collaborate with the RN in making clinical decisions
  • Prioritize and coordinate activities of the nursing unit
  • Be an excellent problem solver and have strong decision-making skills
  • Have demonstrated leadership abilities
  • Have prior experience working in a skilled nursing facility (preferred, but not required)
  • Experience with SigmaCare (preferred, but not required)
