Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Monogram Health Professional Services. As a key member of our integrated Care Team, you will provide high-quality, patient-centered care to individuals with chronic kidney disease and end-stage renal disease.
Our Nurse Practitioners play a vital role in improving health outcomes and quality of life for our patients. You will work closely with our team to develop and implement personalized care plans, conduct comprehensive annual wellness exams, and provide education and support to patients and their families.
As a Nurse Practitioner at Monogram Health, you will have the opportunity to make a meaningful difference in the lives of our patients and be part of a dynamic team that is dedicated to delivering exceptional care.
Responsibilities- Conduct comprehensive annual wellness exams and provide education and support to patients and their families
- Develop and implement personalized care plans to improve health outcomes and quality of life
- Collaborate with our team to coordinate care and ensure seamless transitions
- Prescribe medications and order tests as needed
- Participate in integrated care team meetings and contribute to the development of care plans
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- Ability to work without direct supervision and practice autonomously
- Knowledge of computer-based data management programs and information systems
