About the Role

We are seeking a skilled Nurse Practitioner to join our team at Curo Health Services. As a key member of our Advanced Illness Management team, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities
  • Collaborate with our Professional Services Corporation (PSC) Physician and other palliative team members to deliver advanced illness management and palliative care.
  • Assess patients and develop individualized treatment plans to address their unique needs.
  • Prescribe medications, order laboratory and imaging tests, and provide ongoing care to patients.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in evening and weekend call rotations to provide on-call services.
Requirements
  • Master's degree in Nursing from an accredited school of nursing.
  • Minimum of two years of experience as a Nurse Practitioner, preferably in Family or Adult Medicine.
  • Current license as a registered nurse and Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician and Part B Provider certification.
  • ACHPN certification in Hospice and Palliative Care is preferred.
