Job Summary: Liberty Health is seeking a Patient Services Liaison Specialist to coordinate patient services and develop community outreach programs. The ideal candidate will have a background in healthcare and excellent communication skills.

Duties and Responsibilities:
  1. Perform health care institution and physician office hospice education activities
  2. Coordinate patient services, including admissions, discharges, and transfers
  3. Develop and implement community outreach programs to promote hospice care
  4. Work closely with clinical leadership to ensure smooth operations


Requirements:
  • Bachelor's degree in a related field (or RN, MSW)
  • 2 years of experience in home health/hospice or related field
  • Knowledge of Medicare/Medicaid hospice regulations
  • Ability to travel as necessary


Estimated Salary: $65,000 - $85,000 per year, depending on experience.
