Job Overview

At Roots Community Health Center, we are seeking a skilled and compassionate Health Navigator to join our team. This role is a critical part of our organization's mission to provide high-quality healthcare services to individuals and families in need.

Key Responsibilities:

  • Establish and maintain strong relationships with participants to ensure their comfort and trust.
  • Conduct regular face-to-face visits and outreach calls to assess and address participants' needs.
  • Perform comprehensive barrier assessments to identify and address psychosocial, physical, and mental health barriers.
  • Assist participants in enrolling in public benefit programs and accessing medical and social services.
  • Provide health coaching and support medical providers and clinical staff as needed.
  • Complete all required documentation and reports in a timely manner.
  • Contribute to a safe and clean work environment.
  • Participate in staff meetings, supervision, and training sessions.

Requirements:

  • Personal or family history of systems involvement (jail/prison, foster system, mental health system, etc.) is a plus.
  • 2 years of experience working in non-profit organizations or community clinics is preferred.
  • Comfort level working with diverse communities, including African descent, low-income, unhoused, and individuals with behavioral health challenges.
  • Solid organizational skills, including attention to detail and multi-tasking.
  • Ability to manage time efficiently and communicate effectively.
  • Strong working knowledge of Microsoft Office.
  • Valid California Driver License with a safe driving record.
