About the Role

We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a key member of our pharmacy team, you will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and collaborating with the healthcare team to provide clinical and technical support.

Responsibilities
  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
  • Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure that appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Qualifications
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
