Pharmacist - Clinical Expertise

This position offers the opportunity to grow your pharmaceutical skills under the direct guidance of a state licensed pharmacist while providing essential care to our patients in a closed-door pharmacy setting.
As a Pharmacist with PharMerica, you will be responsible for evaluating physician medication orders, maintaining patient/resident databases, and collaborating with the healthcare team to provide clinical and technical support.

Responsibilities include:
- Evaluating physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
- Maintaining patient/resident databases to include current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensuring appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborating with the healthcare team to provide clinical and technical support as needed.
- Documenting all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Implementing policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Obtaining and maintaining required documentation in accordance with federal and state regulatory agencies and policy.
- Informing the Pharmacy Manager of medication-related errors.
- Assisting facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Qualifications include a Bachelor's Degree in Pharmacy, Pharm D preferred, and one year of experience in long-term care. Licenses/Certifications as required by government regulations and business needs.
