Job Summary

We are seeking a compassionate and skilled Lead Behavioral Health Nurse Practitioner to join our team at Elara Caring.

About the Role

The successful candidate will be responsible for assessing, planning, coordinating, implementing, and evaluating the plan of treatment for patients with behavioral health needs. They will work closely with our multidisciplinary team to ensure high-quality care and make a meaningful difference in the lives of our patients.

Key Responsibilities
  • Assess assigned case load of behavioral health care patients and families to identify physical, psychosocial, and environmental needs.
  • Develop and implement individualized plans of care that address patient-specific needs and goals.
  • Collaborate with healthcare professionals to coordinate care and provide education to patients and families.
  • Counsel and instruct patients and families on healthy behaviors and lifestyle choices.
  • Provide support and guidance to other healthcare professionals as needed.
Requirements
  • Current, unrestricted state RN license.
  • Minimum 1 year of experience in a clinical care setting as a nurse.
  • Able and willing to travel within branch/office coverage area.
  • Dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
  • Able and willing to travel 50% of the time.
